By checking each box below, I acknowledge that I have read and agree to that consent. My agreement applies to every child I list on this form.

Bus Rules & Conduct — I understand the bus rules and policies and what is expected of my child/children while in club and on the bus. I will encourage proper behavior and understand that issues will be handled by leadership per the Welcome Brochure. I understand that non-attendance for three (3) consecutive weeks may allow the reserved seat to be reassigned.

Off-Site Activities — I grant permission for my child/children to participate in off-site activities of Children of God Ministries during club throughout the year, including but not limited to bowling and the Christmas light trip.

Emergency Medical Treatment — In a medical emergency, and if I cannot be reached, I authorize Children of God Ministries leaders to seek and consent to emergency medical treatment for my child/children, including transport to a medical facility. Every reasonable effort will be made to contact me first, and I am responsible for any related medical costs.

Photo & Video Release — I grant permission for Children of God Ministries to use photographs and video for publicity, promotional, and/or educational purposes. A child's first name may accompany a photo, but no last name or address will be included. I waive claims for compensation.
